Unofficial Transfer Form for Google Voice to Google Apps

Lifehacker is reporting some success with a Google form that allows you transfer your Google Voice account to your Google Apps account. The process is officially not sanctioned by Google for this purpose, so you may want to wait, but others are reporting success.
